Programme Details/Care Plan
The Nelson Trust's treatment target is to meet the needs of each client, as identified on their individual care plan.
Care planning is fundamental to the client’s treatment process. The care plan has been designed to meet the guidelines published by the NTA in Care Planning Practical Guide (NTA 2006b) alongside Models of Care: Update 2006. The Nelson Trust care plan is divided into six main headings – drugs and alcohol, physical and mental health, legal issues, social functioning, cultural/ethnic/gender issues and religious/spiritual needs. Every client in treatment will have a different care plan, but typically, targets under each heading would be:
Drugs and Alcohol
Development of healthier coping strategies
- Relapse Prevention techniques and strategies
- Recognition of, and change to, repeating patterns of behaviour
- Reduction of risks associated with substance misuse
Physical and Mental Health
- Improved physical health
- Improved mental health
- Improved self esteem and enhanced self-effectiveness
Legal Issues
- Reduction in offending behaviour
- Improved confidence to manage outstanding legal issues
- Increased awareness of consequences of criminal activities
Social Functioning
Housing
- Safe and appropriate accommodation
- Action Plan to address previous and/or future housing/tenancy issues.
Financial
- Action Plan to address unresolved financial issues
- Budgeting skills
Education Training Employment
- Knowledge of how to access services for college or voluntary work
- Increased confidence in ability to train or retrain
- Action plan to achieve future employment or training aspirations
- Improvement of life skills
Family
- Better understanding of family dynamics
- Improved communication with family and friends
- Improved quality of personal relationships
Cultural / Ethnicity / Gender
- Experience of inclusive practice
- Support with accessing specific foods, products and other relevant issues
Religious / Spiritual
- Support to access places of worship
- Stronger connection to chosen faith/religion
Meeting these targets will prepare clients for life beyond treatment. The targets identified on the care plan aim to support clients by equipping them with the resources they will need in order to take responsibility for their lives.
