Referral and Application Process

First contact with our service is usually made by an individual's care manager or key worker.  However, an individual or family member can apply directly where private funding arrangements are in place.

The Admissions Team will be happy to answer any queries about our programme, how to access the service and current availability of places. They can be contacted on 01453 732861 or 732867.

We endeavour to respond to all enquiries within 24 hours.

Application

All prospective clients must complete an application form, which can be printed off by clicking here. Care managers or key workers may assist in completing this form.

Completed forms should be signed and posted to:
