Assessment
On receipt of a completed application form details are checked by The Admissions Team to make sure all the necessary questions have been answered. Incomplete forms may be returned. The Admissions Team will then contact the care manager or key worker to arrange an assessment date for the prospective client. Assessments are normally conducted face-to-face by a clinically trained member of the assessment team. This is a comprehensive assessment. An initial risk management plan is then devised for the client. The comprehensive assessment normally lasts about an hour and a half.
- Telephone assessments are available on request and in situations where, for example, people are in custody
- Fast track assessments can sometimes be arranged if, for example, an individual is leaving their current agency at short notice.
Occasionally a prospective client may attend a 24-hour assessment. This will be arranged with them and their care manager or key worker when the assessment is booked. In some cases a prospective client may be assessed and then admitted on the same day. Again this will be arranged with the client and the care manager or key worker when the assessment is booked.
Prospective clients are normally advised immediately whether they will be admitted. The Admissions Team will contact the care manager or key worker within 24 hours of the assessment to advise the outcome of the assessment, resolve any outstanding issues where appropriate, such as funding or detox requirements, and make arrangements for the admission. An offer of a treatment place is subject to confirmation of funding and resolution of any outstanding issues. Written confirmation of funding is required prior to a client being admitted.
